6 to 8 points:	High risk with 4.5 to 19.5% 30-day mortality

Calculation Details and Variables

• AgeFactor is assigned values by this ordered logic:
  • If Age ≥90 yr AgeFactor = 21
  • Else if Age ≥85 yr AgeFactor = 15
  • Else if Age ≥80 yr AgeFactor = 14
  • Else if Age ≥75 yr AgeFactor = 9
  • Else if Age ≥65 yr AgeFactor = 5
  • Else if Age ≥55 yr AgeFactor = 3
  • Else if Age ≥40 yr AgeFactor = 1
  • Else AgeFactor = 0.
• Equation parameters such as Sex, have two or more discrete values that may be used in the calculation. The numbers in the parentheses, e.g. (0), represent the values that will be used.
• eGFRFactor is assigned values by this ordered logic:
  • If eGFR CKD Epi <30 mL/min/1.73 m(2) eGFRFactor = 3
  • Else if eGFR CKD Epi ≤59 mL/min/1.73 m(2) eGFRFactor = 2
  • Else eGFRFactor = 0.
• The O2SatFactor defult value is 0 points. If the subject is 50 years old or younger, and the O2 Saturation is 93% or lower, O2SatFactor is assigned 1 point. For subjects older than 50 years, the O2SatFactor is assigned 1 point when the O2 Saturation is 93% or lower.
• The default NeutLymphRatioFactor is 0 points. If the Neut Lymph Ratio >6.33 the NeutLymphRatioFactor is assigned 2 points. Else if the Neut Lymph Ratio ≥3.22 and ≤6.33 the NeutLymphRatioFactor is assigned 1 point.
