- Can the subject carry out normal activity and work?
- Does the subject feel normal with no complaints or signs of disease?
- Are there minor signs or symptoms of disease?
- Does normal activity require effort with some signs or symptoms of disease?
- Is subject unable to work, but can live at home with some assistance?
- Can the subject care for self but is unable to carry on other normal activities?
- Does the subject require occasional assistance but can care for most personal needs?
- Does the subject require significant assisstance, including frequent medical care?
- Is the subject unable to care for self?
- Is the subject disabled, such that special assisstance and care is required (short of hospitalization)?
- Is the subject severly disabled, requiring hospital admission but subject is not moribund?
- Is the subject very ill, requiring hospitalization with actve supportive care?
- Is the subject moribund?
