This calculator evaluates the following Clinical Criteria:
- Site of primary tumor
- Pre-chemotherapy platelet count ≥350,000/microL
- Hemoglobin level <10 g/dL or use of ESAs
- Pre-chemotherapy WBC >11,000/microL
- BMI ≥35 kg/m2
