  Sequential Organ Failure Assessment (Quick): qSOFA Score

Input

Glasgow Coma Score
Systolic BP
Respiratory Rate

Results

qSOFA Score

Include literature references: Yes   No

  qSOFA Interpretation

2 to 3 points: High risk
0 to 1 point: Not high risk

Notes
  • *A Glasgow coma scale score of 15 requires all the following: Spontaneous eye opening; accurate orientation to person, place, and time; and appropriate motor responses to commands. If this score is less than 15 gcsp is set to 1.
  • For this calculation, rrp = 1 when the respiratory rate is ≥22 breaths/min, and 0 for slower rates. sysbpp is set to 1 when the systolic blood pressure is ≤100 mmHg, and 0 for higher blood pressures.

Equations used

qSOFAScore = gcsp + rrp + sysbpp
