This calculator evaluates the following Clinical Criteria:
- Stays at home most of the time because of back
- Must change position frequently for back comfort
- Gait slower than usual because of back
- Not doing any of the usual jobs around house
- Must use a handrail to get upstairs
- Lies down to rest more often
- Must hold on to something to get out of a chair
- Tries to get other people to do things
- Gets dressed more slowly then usual
- Can stand only short periods of time because of back
- Avoids bending or kneeling down
- Has difficulty getting out of a chair because back
- Back is painful almost all the time
- Difficulty turning over in bed because of back
- Appetite reduced because of back pain
- Trouble putting on socks because of back pain
- Walks only short distances because of back
- Sleeps less well because of back
- Dresses with help from someone else because of back
- Sits most of the day because of back
- Avoids heavy jobs around the house because of back
- More irritable and bad tempered with people
- Goes upstairs more slowly due to back
- Stays in bed most of the time because of back
